If you would like to have your immunization records accessible in your MyIR Mobile account but had received them elsewhere, such as another state, country, a government facility, or childhood vaccinations, we recommend the steps below as a guideline to get them reported to the state registry or Immunization Information System (IIS).
If you already have a copy of your record(s):
- Take these to your local state healthcare provider. Per their discretion, they have the authority to enter those into the state registry or IIS on your behalf.
If you still need to obtain a copy of your record(s):
- Contact the previous healthcare provider or pharmacy and request a copy of your records; or contacting the organization that has your records in their system so you may obtain a copy and have them entered in the state registry or IIS.
- If the healthcare provider or organization you need to contact is inaccessible, you may try to contact the Department of Health where you had your vaccinations. See the link below:

If you do not yet have a healthcare provider in your current state, you may reach out to any county health department near you and have them report any records requested. We recommend to research what you will need to bring with you to verify your identity with them beforehand.
