If you moved to a new state, you are still able to access your MyIR account and the immunization records linked to that account. However, to create a record in your new state immunization registry, please print your records and give a copy to your new healthcare provider.
Vaccinations you have received in another state may not have been transferred to your present state’s immunization registry. Typically, if you have a copy of your immunization records from your healthcare provider in another state, you can provide this information to your current healthcare provider. They can often add that information to your records within your current state’s immunization registry.
If you do not have these records, or are unable to obtain copies from your previous healthcare provider, you may contact the Immunization Program of the state health department in your former state. Many state health departments have ways to provide you with an official record as long as you are able to meet their verification requirements.
