MyIR Mobile is designed for consumers who would like access or need accessing their own personal immunization records for themselves and their family. MyIR Mobile Support is not involved in the process of releasing immunization records in any other situation.

I have a record release form and I don't know where to submit it.
Those forms usually contain such instructions on the form. Instructions such as where to fax/send it to after you've completed it. For specific guidance you may contact the provider helpdesk phone/ or chat listed on the left side under the menus of your IIS's webpage.
