You can access your state reported immunization records through your MyIR Mobile account, which is separate from your Zendesk Support account.

- If you don't have a MyIR Mobile account yet, you can click here to register.
Once you have logged into your MyIR account, you will be directed to a screen that resembles the one shown below. To access your reported immunization records, simply click on the "Immunizations" tab.
Q. What if it says, "You will see your immunization history and needs once you are linked?"
A. You'll need to try to get your immunization records linked first. Click Try Again and begin the verification process.
Q. What if it says "!Record not found?"
A. It seems that there is a slight mismatch between the information you provided and what has been reported to the immunization registry on your behalf. However, there's no need to worry! If you are confident that your information is correct and up to date, we are here to help you. To contact MyIR Mobile Support, click on the Athena Bot in the bottom right corner, then click on Get Help Now. You also have the option to submit a Support Request here and our team will contact you via email to help.
