Any name or demographic corrections will need to be done with your provider, clinic, or local county health department. They are the only ones authorized to make these changes. The way your documents for the certificate or QR code generate with your first and last name information is from how your provider, clinic or pharmacy reported it to the state. Middle names do not generate on these documents because it is optional for users.
If you would like to see your middle name listed on your documents then you will need to reach out to your provider, clinic, or local county department of health and request that they report your middle name along the first name field. Once they have reported this change, it can take up to 7 days for that information to flow from each system and finally appear in your MyIR Account.
As a reminder, make sure the information in your account is as exactly as you notified your provider or pharmacy.
